Difficulty in clinical identification of neonatal seizures: an EEG monitor study.

چکیده

Seventeen newborns were monitored for 24 hours using a three-channel ambulatory EEG (A/EEG). All newborns were thought to be having subtle seizures by the nursery staff. Fifteen of the 17 newborns were recorded as having 1-30 clinical seizures during the time of monitoring. Only one newborn had clinically identified seizures associated with A/EEG discharges. The seizures were characterized by eye rolling. Fifty-two episodes (thought to be seizures) of lip smacking, bicycling, jerking, fisting, staring, stiffening, or any combination of the above occurred in eight newborns without an associated discharge on A/EEG. However, two of the eight had seizure discharges at other times, not associated with any clinical manifestation. Seventy-four apnea spells, thought to be possible seizures, occurred in seven newborns. None was associated with discharges on A/EEG, but one of these newborns had 50 A/EEG discharges unrelated to apnea or other clinical manifestations.

The diagnosis and treatment of neonatal seizures.

The occurrence of neonatal seizures is an important clinical sign indicating brain disorder in neonates. An identification of neonatal seizures is critical in the management of high risk neonates. However, the diagnosis and management of neonatal seizures are challenging, because electroclinical dissociation is an outstanding feature of neonatal seizures. Neonatal seizures are frequently not ac...

Application of an Amplitude-integrated EEG Monitor (Cerebral Function Monitor) to Neonates

Amplitude-integrated EEG (aEEG) is an easily accessible technique to monitor the electrocortical activity in preterm and term infants in neonatal intensive care units (NICUs). This method was first used to monitor newborns after asphyxia, providing information about future neurological outcomes. The aEEG is also helpful to select newborns who benefit from cooling. The aEEG monitoring of preterm...

Clinical and EEG response to anticonvulsants in neonatal seizures.

During a two year period prospective continuous electroencephalographic (EEG) monitoring of 275 infants identified seizure activity in 55 cases, 31 of whom were treated with anticonvulsant drugs on clinical grounds. EEG and clinical response was complete in only two and equivocal in another six. Clinical response with persistent EEG seizures occurred in 13 and neither clinical nor EEG response ...

Non-expert use of the cerebral function monitor for neonatal seizure detection.

BACKGROUND The cerebral function monitor (CFM) is widely used to detect neonatal seizures, but there are very few studies comparing it with simultaneous electroencephalography (EEG). OBJECTIVE To determine the accuracy of non-expert use of the CFM and to assess interobserver agreement of CFM seizure detection. PATIENTS Babies admitted to the neonatal intensive care unit at King's College Ho...

An evaluation of automated neonatal seizure detection methods.

OBJECTIVE To evaluate 3 published automated algorithms for detecting seizures in neonatal EEG. METHODS One-minute, artifact-free EEG segments consisting of either EEG seizure activity or non-seizure EEG activity were extracted from EEG recordings of 13 neonates. Three published neonatal seizure detection algorithms were tested on each EEG recording. In an attempt to obtain improved detection ...
